Please complete the following form to be added to the VIP list. You must show a valid local Hawai'i ID at the door to confirm your kama'aina status. Other I.D. includes Hui Card, Flight Attendant ID, Hotel Employee ID, Military ID (Active), Yellow Card, & Blue Card.
